In this article, you will find comprehensive instructions on how to complete the Customer Billing Information page for creating an application form. This is step two (2) in the process.

Definitions:

Instructions:

1. Select Who Should Be Billed

  • In the Customer Billing Information section, you will need to select who should be billed by clicking on the checkbox for either the Generator or the Third-Party Company.
  • Some of the fields may auto-populate based on information entered in the previous section, but make sure to double-check that everything is correct.

2. Enter Billing Information

  • Type to fill out the required fields for Billing Company, Billing Address, Billing Contact, and Billing Phone.

3. Optional Fields

  • PO Number: Enter the PO Number in the appropriate field if applicable.
  • AFE: Enter the AFE number if available, for tracking billing expenditures.
  • EDI: Add the EDI Number if required.

4. Signatory Information

  • In the Signatory Name field, enter your name, along with your Phone Number and Email. This person is responsible for signing off on the application form.
  • Secondary Signatories: If applicable, you can add up to two additional signatories by entering their name, email, and phone number.
  • Tertiary Signatory: You may also add a third signatory if necessary.

5. Add Notes 

  • If there are any additional notes or instructions for the billing department, enter them into the provided Notes field. These notes can help the team when they are creating your invoice.

6. Proceed to the Next Section

  • Once the Customer Billing Information section is completed, click 'Proceed' to continue to the next part of the application form.
