Instructions:

  1. Create an Application Form

    • Navigate to the WiQ Main Page.
    • Click on '+ Create New Application Form'.
    • The application form will open and you can begin entering the required information.
  2. Complete Generator Information

    • For detailed information about the Generator Information section of the application form creation process, check out: Generator Information
    • The Generator Information section will auto-populate with the company name, phone number, address, city, and province, which are greyed out and cannot be changed.
    • Type in your name into the 'Representative’s Name' field.
    • Type to enter your phone number and email into the corresponding fields.
    • If you are working with a third-party company, click the toggle to enable additional fields.
      • Fill out the third-party’s information if applicable.
    • Once completed, click 'Proceed' to move on to the next section.

  3. Complete Customer Billing Information

    • For detailed information about the Billing Information section of the application form creation process, check out: Billing Information
    • In the Customer Billing Information section, you will need to select who should be billed—either the generator or the third-party company. Type to enter the required fields for Billing Company, Billing Address, Billing Contact, and Billing Phone.
    • Optional Fields: You can also enter an optional PO Number, AFE, or EDI number in the appropriate fields. Additionally, provide your name in the Signatory Name field, along with your phone number and email.
    • Secondary Signatories: If necessary, you can add secondary or tertiary signatories by entering their name, email, and phone number. You can also choose to add any relevant notes in the provided text box. 
    • Click 'Proceed' to move on to the next section. 
  4. Add Transporter Information

    • For detailed information about the Billing Information section of the application form creation process, check out: Transporter Information
    • In the Transporter Information section, select the appropriate transporter from the dropdown menu. The details of the selected transporter will automatically populate the fields.
    • Click 'Proceed' to continue to the next section.
      WiQ Tip! If you do not see the transporter you need, contact your WiQ superuser or reach out to the WiQ team via chat or phone for assistance.

       


  5. Complete Source Location Information

    • For detailed information about the Source Location Information section of the application form creation process, check out: Source Location Information
    • In the Source Location Information section, select the location type from the dropdown menu. Based on your selection, different fields will be required.
    • Location Type Options:
      • Wellsite: Fill out the Surface Location, UWI, License Number, and any Cost Codes. You can also add additional UWI lines by clicking the '+' sign, or remove them by clicking the '-' sign. 
      • Facility: Enter the Facility Name, License Number, Surface Location, and the Province.
      • Pipeline: Select Lat/Long or LSD from the dropdown and enter the required information.
      • Other: Choose Street Address from the dropdown and enter the address and postal code.
      • Once the source location information is filled out, you can select the Waste Generation Process from the dropdown and click 'Proceed' to continue to the next section. 

  6. Fill Out Waste Information

    • For detailed information about the Waste Information section of the application form creation process, check out: Waste Information
    • Select whether the waste is Oilfield or Non-Oilfield waste from the dropdown list.
    • Description and Classification: Enter a detailed description of the waste in the Waste Description textbox. Then, select the Waste Classification from the dropdown (either DOW/HAZ or NON-DOW/NON-HAZ).
    • Additional Waste Details:
      • Enter the Estimated Volume of waste and select the Unit of Measure from the dropdown list.
      • Choose the Generation Frequency and Shipping Type from their respective dropdowns.
    • Properties of Concern: Select the Properties of Concern by checking the appropriate boxes. If the waste has any special handling instructions, type them into the provided field. You can also attach an analytical report or any supporting files. 
    • Click 'Proceed' to continue to the next section.
       
  7. Add Receiver Information

    • For detailed information about the Waste Information section of the application form creation process, check out: Receiver Information
    • In the Receiver Information section, select the primary receiver site location from the dropdown list. The company information for the selected receiver will auto-populate.
    • Secondary Receiver Site: If applicable, you can select a Secondary Receiver Site from the dropdown list.
    • Click 'Proceed' to continue to the next section. 

  8. Review and Submit the Application

    • For detailed information about the Review section of the application form creation process, check out: Review and Sign
    • On the Review Page, you will be able to see all the information you have entered into the application form. Review this information thoroughly. If you need to make any changes, click on the specific section in the sidebar to make edits. You can also add notes in the Notes field.
    • Sign Off and Submit: Once everything is reviewed, sign off on the ticket by typing your name into the 'Name of Authorized Person' field. Check the box to confirm that all information is correct, and then click 'Complete'.
      A Generator Declaration will pop up. The declaration summarizes that you are responsible for the waste material's classification and compliance. After reading through it, click 'Confirm' to proceed.
    • Final Submission: Click 'Proceed' to finalize your submission. You will see a confirmation pop-up detailing the next steps in the application process. After reading the confirmation, click 'Complete'.
    • The application form will now be in 'Submitted for Approval' status. It will remain in this status until the receiving site approves it, and it becomes an 'Active Material Approval'.
