• The Landfill Application Form Process involves seven (7) steps, each requiring you to complete a section of information to ensure a compliant and complete application form. These sections include:     
      1. Generator Information
      2. Billing Information
      3. Transporter Information
      4. Source Location Information (This Article)
      5. Waste Information
      6. Receiver Info
      7. Review and Sign

Instructions:

1. Select the location type
First, go to the Source Location Information section of the application form. From the dropdown menu, select the type of location for the waste generation source. Available options include Wellsite, Facility, Pipeline, and Other. Based on your selection, different fields will be displayed for you to complete.

• Wellsite
  If you choose "Wellsite," you will need to enter the Surface Location, UWI, License Number, and any applicable Cost Codes. To add additional UWI lines, click the '+' sign. If you need to remove a UWI line, simply click the '-' sign.
• Facility
  If you select "Facility," the required fields will be Facility Name, License Number, Surface Location, and the Province. Ensure all required fields are filled in accurately.
• Pipeline
  If you select "Pipeline," you will need to choose whether to provide Lat/Long or LSD coordinates from the dropdown. Enter the necessary details according to your selection.
• Other
  If "Other" is selected, you must choose Street Address from the dropdown and enter the address and postal code.

2. Complete the location details
After selecting the location type and filling in the required fields, ensure all the details are correct. Once this is completed, navigate to the Waste Generation Process dropdown, select the appropriate option, and click 'Proceed' to continue to the next section.
